Patient Administrations

The Patient Administration Division at Ireland Army Health Clinic, Fort Knox MEDDAC, is located on the 2nd floor.

Outpatient Medical Records

Medical Records are the property of the United States Government. The Outpatient Medical Records Department staff have the responsibility for ensuring all medical records held in our custody are safeguarded from loss or tampering, and that the information in the records is released only to authorized individuals. We are dedicated to providing the medical information our customers need while protecting their privacy. Our goal is to deliver a quality product in a prompt, professional manner, in accordance with military regulations, and state and federal laws. We are here to serve you!

Requesting Information

Patients may request copies of their medical records by completing a DD Form 2870 (Authorization for Disclosure of Medical or Dental Information) and submitting it by fax, by encrypted email or by visiting the Outpatient Medical Records Section located on the 2nd floor between 7:30 a.m. and 4:00 p.m. on weekdays, excluding federal holidays. The Outpatient Records Section closes at 3:00 p.m. for training on the 2nd and 4th Wednesday of the month. A valid ID to verify signature is required to request a copy of your medical records. Additional copies will incur an administrative fee and charges per page or per CD. Please allow up to 30 business days for processing. Behavioral Health records must be reviewed and possibly cleared by a provider before release. Beneficiaries are entitled to one free personal copy of their medical record, per AR 40-66.

Military Health records can be requested from the nearest facility to where you live. You do not need to request them from the last facility where you were seen.
