    To change Providers, call Humana or visit https://www.humanamilitary.com/beneficiary/benefit-guidance/quick-access/change-your-pcm

    Life happens and health conditions change. If your conditions are outside of the scope of practice of your PCM, Ireland Clinic changes your PCM Provider based on acuity.
